More specifically, its the fact that the current design sports a chest to its body, which this version doesn't have, just the stomach part.
@piggi5x: These Tortletummy sprites are unused since you don't see different-sized Tortletummies in the game.

i could have sworn you fought this guy in one of the underwater levels? why is it in the unused section
