Probably bleeding after all the dangerous final stage encounters.
Why does he have a red mouth in his capsule sprite when all his other sprites have a black mouth?
I had a look in the ROM and I noticed an error with the "look up" cutscene sprite. The top two tiles are misaligned and should go 2-3 pixels to the right. Same with one of the balancing frames (though here the misalignment is at the chest), which was also missing one black pixel on the left leg.
The vertical hang move uses one more frame which is a different combination of the arms, body and goggles.
Finally there's a missing burned/backfire sprite while hanging.
