The calzone was intended for the unused mansion level. While not used in any of the levels, it does actually make a brief appearance at the end of the game with the other Secret Treasures when they're used to resurrect Pillar John.

was probably for the mansion, which was the last to be cut and would've come between pizzascare and don't make a sound.
Is the calzone unused? I've never seen it, placement suggests it was for world four. If it is unused, I'm guessing it may have been for kung-fu or dragon's lair since kung-fu was for world four, and dragons lair had a cheese dragon.
Edit: Now that I think about it, the nigiri was probably for kung-fu, and now gets used for the pig city. If anyone knows what the calzone is for, please let me know.
