So either he fell down of the ladder or he... Fell Down.
considering the fact that in the Undertale alarm clock with Asgore we learn that the Rudy in that universe Fell Down at some point before the game (he probably lived longer in Deltarune due to access to better healthcare on the surface), and that Deltarune appears to be a few years later just in an alternate world... I don't think he fell of a ladder, besides Toby knows what he's doing when it comes to storytelling so he wouldn't use the term "Fallen Down" if he had just fallen off the ladder.
