I found the disc cleaning kit online that's the OEM one, and the disc itself looks like the icon. Too bad the lowest price is $80, and tends to go to around $160 for a CIB or sealed one.
There is a disc cleaning kit for the Wii u and that icon would pop up when you inserted the disc into the console. As far as I know, these are region free. Haven't been able to find a video on the internet yet showcasing this disc though.
In the Wii U Menu files, the image is called AppIconDiscCleaner.tga
Maybe Gamecube games considering the small disc?
